We remove a tooth only when keeping it would cause more harm than losing it — and when we do, we plan the space afterward so your child's bite develops the way it should.
When a Tooth Extraction for Kids Is the Right Call
Our strong preference is always to save a tooth. Baby teeth guide the permanent teeth into place, so removing one early has consequences we have to plan around.
That said, there are situations where removal is clearly the better outcome for your child.
- Decay too extensive to restore with a filling or crown
- Infection or abscess that has not responded to treatment
- A baby tooth that will not fall out on its own and is blocking the adult tooth
- Severe damage from a fall or sports injury
- Overcrowding, where an orthodontist has recommended removal to create room
- Extra teeth that are pushing the normal teeth out of alignment
What We Do About the Empty Space
Removing a baby tooth early leaves a gap, and neighboring teeth will drift into it. Planning for that gap is as important as the extraction itself.
Space Maintainers
A small custom appliance that holds the gap open until the permanent tooth arrives. This is our most common follow-up after an early baby molar extraction.
Monitoring Only
If the permanent tooth is nearly ready to erupt, no appliance is needed. We simply watch it at your regular six-month visits.
Orthodontic Coordination
When an extraction is part of a braces plan, we work directly with your orthodontist so the timing supports their treatment rather than complicating it.
Saving the Tooth Instead
Before we extract, we always ask whether a pulpotomy and crown could save it. Frequently they can, and we will tell you when that is the better path.
The Extraction Visit, Step by Step
Knowing the sequence removes most of the fear — for your child and for you. Here is exactly what happens.
Step 1: Numbing, Done Right
We apply a topical gel first so your child never feels the injection itself. We then take our time with the local anesthetic and wait until the area is fully numb before touching anything. Rushing this step is the main reason extractions go badly, so we do not.
Step 2: Comfort Options
Nitrous oxide is available for most children and takes the edge off nicely. For very anxious children or complex extractions, we can discuss oral or IV sedation in advance.
Step 3: The Extraction
Your child will feel firm pressure and hear some sound, but no pain. A baby tooth typically takes under a minute. We narrate as we go or stay quiet, whichever your child prefers.
Step 4: Gauze and Going Home
Your child bites on gauze for about thirty minutes to control bleeding. We send you home with written instructions, and we call to check in that evening.
